By integrating body, mind, and heart, this organization strengthens communities and addresses the social determinants of health. As part of the Federally Qualified Health Center movement, it provides comprehensive primary and preventive healthcare services.

With a team of over 350 professionals, Union Community Care has served more than 40,600 neighbors through 149,100 healing visits, promoting whole health and equity.
