Our Story
Together We Can Foundation-Smart Transitions is dedicated to improving outcomes for at-risk youth in Hampton Roads and Southeastern Virginia. By providing essential life skills and workforce readiness programs, the foundation ensures that young individuals are better prepared for independent adult life.
Impact
In FY 2023-24, 1,392 youth benefited from direct programming, contributing to a total of 11,297 youth served since 2010. The foundation boasts high success rates in educational attainment and employment for its participants.
