Our Story
The Staten Island Performing Provider System (SI PPS) is a dedicated non-profit public health organization established in 2014 under the New York State Department of Health Medicaid 1115 waiver amendment. SI PPS was created to build a robust network of medical, behavioral health, and social service agencies, all focused on enhancing health outcomes and reducing costs for the Medicaid program.
Through innovative projects, SI PPS has successfully met state targets for reducing avoidable hospital utilization and improving access to essential medical and behavioral health services. The high performance funding earned at the end of the program has been reinvested into network partners, allowing for the expansion of vital population health improvement initiatives.
In 2024, SI PPS took a significant step forward by becoming the lead entity in Richmond County to develop a Social Care Network. This initiative aims to enhance the capability of the network to screen for health-related social needs, improve coordination of social care services, and ultimately reduce health disparities in the community.
