Founded in 1990 by Dr. Ralph Montgomery, Salt Lake Donated Dental Services (SLDDS) addresses the critical need for dental care among low-income individuals in Utah. With a focus on preventive and restorative services, SLDDS operates two key programs: the Donated Dental Program, offering free services to those at or below 100% of the Federal Poverty Level, and the Discounted Dental Program, providing affordable care for individuals at or below 200% of the Federal Poverty Level. Located in Glendale, Salt Lake City, SLDDS serves approximately 4,900 patients each year.
