Hospice Care Team Beaumont

Our Story

Since 1983, Hospice Care Team has been dedicated to delivering Hope, Help, and Comfort to patients and families in Galveston and the surrounding counties. With a mission to maintain the highest level of independence for patients in familiar surroundings, HCT provides a professional team of physicians, nurses, social workers, hospice aides, chaplains, and volunteers. This compassionate care ensures pain relief and symptom control while offering support to families through active bereavement counseling. In 2017, HCT expanded its reach by acquiring Heart of Texas Hospice, now known as Hospice Care Team Beaumont, allowing even more individuals to receive the care they deserve.
