Families in Transition is dedicated to preventing and breaking the cycle of homelessness by providing dignified housing and comprehensive solutions. With headquarters in Manchester, NH, and additional locations in Concord and Wolfeboro, the organization operates over 25 facilities across the state, offering clean, safe, and affordable living for individuals and families.

Impact

In 2023, Families in Transition served 14,457 individuals, provided groceries to 3,700 households, offered emergency shelter to 1,055 individuals, and delivered substance use treatment to 374 individuals, showcasing their significant impact on the community.
