Founded in 1979 in Fallbrook, California, The Foundation for Senior Care has a rich history of evolving to meet the healthcare needs of the community. Originally established as the Fallbrook Hospital Foundation, the organization shifted its focus in 1998 to address the growing needs of seniors and adults with disabilities. After extensive discussions with local healthcare providers and community organizations, it became clear that there was a pressing need for caregiving services for adults with dementia and other memory-loss issues.

In response, the Foundation opened an adult day care facility in 2000 and launched a resource center offering technology education for seniors. Over the years, the Foundation has continued to adapt its services, introducing the Care Van Transportation program in 2004 and the Support and Referral Advocacy program in 2005. In 2008, the organization rebranded to The Foundation for Senior Care to better reflect its mission of supporting seniors and adults with disabilities.
