Our Story

Continuum of Care, Inc. is dedicated to enabling individuals facing challenges such as mental illness, intellectual disabilities, addiction, or homelessness to rebuild meaningful lives and thrive within their communities. Operating across Connecticut, the organization manages over 60 group homes and crisis centers, providing essential services and support to those in need.

Impact

Each year, Continuum of Care, Inc. successfully assists over 500 individuals in breaking the cycle of homelessness or institutionalization. This includes more than 100 veterans who transition from difficult pasts annually. The organization also answers 5,000 calls through its warm line, offering vital support to those in crisis.
