This innovative CCN Network offers affordable access to a medical home, ensuring that those in need receive both primary and specialized care.
Before the Affordable Care Act (ACA) was enacted, CCN made healthcare accessible for just $10 a month, allowing patients to choose a medical home that met their needs. Today, many individuals, particularly in immigrant communities who remain ineligible for ACA coverage, still rely on CCN for essential medical services. The organization has also become a national leader in enrolling individuals into ACA plans, successfully registering thousands in Chicago’s at-risk neighborhoods.
