Our Story

Founded in 2016 by leaders from nine Federally Qualified Health Centers (FQHCs) in Massachusetts, Community Care Cooperative (C3) is the first organization of its kind. Established in response to the redesigned Medicaid program, C3 serves as a united voice for health centers, advocating for their needs on a national scale. As the only Accountable Care Organization (ACO) in Massachusetts governed by FQHCs, C3 is the largest FQHC-owned organization in the country, dedicated to enhancing primary care and advancing racial justice.
