Our Story

Avenues to Community is a dedicated non-profit organization that has been providing essential Case Management and Service Coordination services to individuals with disabilities since 2001. With a strong partnership with the Dane County Department of Human Services, the organization focuses on supporting children and youth eligible for the CLTS Medicaid Waiver Program.
