Our Story
Avita Home Health & Hospice, formerly known as HomeCare Matters, was established through the merger of Tri Hospital Home Health Care and Crawford County Hospice. Since 1985, this organization has been dedicated to delivering high-quality in-home care, becoming a wholly owned subsidiary of Avita Health System in 2017 and rebranding in 2020.
